Premier Homecare Services North York strives to ensure your needs and those of your loved ones are always met. You can depend on our service and rates to make that possible.
Free In-Home Assessment
Free in-home assessment consultations are conducted in order to properly understand your situation and needs. An owner, client care co-ordinator or supervisor will come to visit your home, allowing you to meet the people who will be supervising your care.
Schedule about one hour for a consultation. You’ll be asked important questions about health in order for us to understand your needs. This will allow us to create a customized care plan just for you. Feel free to invite family and friends to attend, they can provide comfort and ask questions.
Making sure we understand you, your medical conditions, and your likes and dislikes is very important.
Some questions we may ask:
- Do you have any allergies?
- Have you had any falls recently?
- Do you need help bathing?
- Do you prefer a bath or shower?
- Do you need help getting dressed or undressed?
- What medications do you take?
- What activities do you enjoy?
Once the questions are answered, a Home Safety Assessment can be done to ensure precautions are taken to make your home as safe as possible. The goal is to reduce risk of falls caused by loose rugs or mats, and ensure the safety of your kitchen, bathroom and bedroom.
No Obligation Service Agreement
After the assessment, we will create a care plan based on your requests, budget, and needs. With a goal of providing you with the right care and allowing you to maintain your independence, this is YOUR care plan. If you don’t like what we’ve planned out or want to change something, please tell us. We want to keep those lines of communication open with our clients to provide them with the best service possible. Your feedback is important.
Once a decision has been made that we are the home care company for you, a Service Agreement will be signed. The agreement is not a contract, it simply outlines the details regarding your services and provides you with our policies and procedures. The Service Agreement can be cancelled at any time. All we ask is that you give us a reasonable amount of time to get settled in, we believe that to be a few weeks depending on the frequency of your service. We believe that our service will speak for itself.
Making sure that you like your caregiver and get along with them is something we work hard to achieve. Sometimes, adjustments need to be made. Do not hesitate to request a different caregiver. We’ll set up a meeting to address concerns and choose another caregiver better suited to you.
There is always someone available to help you if needed. You and your family can have peace of mind knowing a representative can answer your calls no matter the time of day. This added security is a measure of the quality service we provide.
Our flexible hours allow you to start building a care plan with as little as 3 hours all the way up to 24 hours. Services can be conducted once a week, once a month, or whenever you’d like, the choice is yours. Service is available 24/7 and a caregiver can even live in your home with you to provide around-the-clock care.
